Client Waiver and Acknowledgment

The undersigned, hereinafter referred to as the "Client," acknowledges and agrees to the following terms regarding the non-medical and elective ultrasound services provided by Bump.Baby.Bliss., LLC:

1. Non-Medical Nature of Services: The ultrasound services and imaging provided by Bump.Baby.Bliss., LLC are purely elective and non-medical. They are intended solely for entertainment purposes and are not to be used for medical diagnosis, treatment, or evaluation of any medical conditions. The Client acknowledges that these services do not determine or assess the health status of the fetus(es) or the Client during pregnancy or at birth.

2. Entertainment Only: The Client understands that the images and videos obtained from these ultrasound services are for entertainment purposes only. They are not to be relied upon for any medical diagnosis or treatment of the fetus(es) or the Client. Bump.Baby.Bliss., LLC staff are trained by Probo Medical but are not licensed medical professionals or sonographers'.

3. Medical Care: The Client affirms that they are under the care of a qualified physician regarding their pregnancy. Bump.Baby.Bliss., LLC does not provide medical oversight or evaluation.

4. Limitation of Liability: The Client agrees that Bump.Baby.Bliss., LLC, including its staff, agents, and employees, shall not be held liable or accountable for any consequences arising from known or unknown medical conditions of the Client or the fetus(es). This includes failure to diagnose or inform about any medical conditions or risks.

5. No Guarantee of Quality: The Client acknowledges that Bump.Baby.Bliss., LLC makes no guarantee regarding the quality or content of the ultrasound images or videos. Factors such as fetal position, placenta location, and maternal anatomy can affect image quality, and Bump.Baby.Bliss., LLC has no control over these factors.

6. Release of Liability: Under no circumstances shall Bump.Baby.Bliss., LLC, its staff, agents, or employees be held liable for any injury or damage that may occur to the Client or the fetus(es) as a result of the ultrasound performed. By signing this waiver, the Client releases, acquits, holds harmless, and forever discharges Bump.Baby.Bliss., LLC from any and all claims, demands, damages, actions, or causes of action, whether foreseen or unforeseen, resulting from the ultrasound services provided.

7. Acknowledgment and Understanding: The Client certifies that they have carefully read and fully understand this liability waiver and release. The Client voluntarily agrees to the terms set forth herein.

By signing this document, the Client confirms their understanding and acceptance of the above terms and conditions.
